Initial Consultation

Comprehensive 60-minute initial consultation review of medical history, lifestyle, goals, and risk factors, with personalized recommendations based on longevity science, advanced diagnostics, and clinical insight (in-person or virtual). Health insurance can be used for standard services (labs, imaging), otherwise labs and imaging can be ordered through our office at a discounted rate.

Investment: $300 for consultation & lab/imaging orders.

After your initial consultation, you can choose which membership is best to support your lifestyle— All Longevity memberships require a 12-month commitment.

GOLD Longevity

Includes Health and Biological Age Assessments, Access to Direct Provider Messaging, Medication Management, 15% savings on Peptides & Supplements, and three (3) Skilled Provider Sessions. Includes 170+ point executive physical exam with EKG, pulmonary function testing, and gold-standard body composition analysis, along with establishing care and six months of prescription rights for medications prescribed during the initial consultation.

Investment: $5,000 paid in full or $2,000 initial payment with $1,200 every 12 weeks.

DIAMOND Longevity

Includes Health and Biological Age Assessments, Access to Direct Provider Messaging, Medication Management, Peptides and Supplements, and six (6) Skilled Provider Sessions. Includes 170+ point executive physical exam with EKG, functional health measures, pulmonary function testing, VO₂Max testing, and gold-standard body composition analysis, along with establishing care and 12 months of prescription rights for medications prescribed during the initial consultation.

Investment: $10,000 paid in full or $4,300 initial payment with $2,500 every 12 weeks.

BESPOKE Longevity

White-glove longevity partnership with 24-hour access to a medical professional who curates services and interfaces with outside providers on your behalf. Includes hormones, peptides, supplements, all labs, whole body MRI two times per year, and more. Custom quotations available.

PEPTIDE Membership

Comprehensive 60-minute initial consultation review of medical history, lifestyle, goals, injuries, and risk factors, with personalized recommendations based on peptide, longevity & preventative science, and clinical insight (in-person or virtual). After your initial consultation, you can choose to continue a monthly Peptide Membership, which gives you access to obtain medical grade peptides & syringes from our office at a discounted rate, as well as a full plan for supplements for health & wellness optimization. Health insurance can be used for standard services (labs), otherwise labs can be ordered through our office at a discounted rate. Peptide Membership requires a 6-month commitment. This Peptide Membership does not include the GLP's for weight loss.

Investment: $300 for initial consultation & lab order.

After initial consultation you can choose investment of $120.00 per month, or $300 every 12 weeks.

Secure, convenient messaging with your provider plus easy access to results and insights anytime through the Tebra Patient Portal.

Medication Management

Guidance on prescription and non-prescription medications, supplements, and hormone optimization. Includes assistance with insurance prior authorizations and coverage requests for the most cost-efficient options.

Longevity Specific Medications

While there are no specific medications made for longevity, there are some prescription medications that can be used off-label under the care of a healthcare provider to help reduce the risk of disease, improve overall health, and even potentially extend life. Some of those medications include rapamycin, semaglutide, tirzepatide and metformin, along with a multitude of non-prescription medications and supplements which can help longevity and healthspan applications.We assist our patients to navigate the many challenges and barriers to sourcing and obtaining longevity prescription & non-prescription medications. We do extensive research to identify the safest and most effective options backed by the latest research and personalize a regimen adapted specifically to your goals and your health.
